I was passed these two permission slips below, and yes I see the humor. As a married man, I’ve run into the gauntlet of requirements at times. But I’m glad to see I’m not alone as evidenced in this form. No I did not create them, and yes I’m sure my wife would kill me if I presented her one of these.
